Clerical Associate – Temporary Part-Time (Line: PCA-8)

Position summary:

The Clerical Associate provides support to the patient care team. He/she also acts as a navigator for patients and families. Complete coordination duties to ensure the patient care is organized and standardized.  He/she is a key contact for patients, substitute decision makers (SDMs), LHIN, Patient Flow team, lab team, healthcare providers, and community partners.

Responsibilities include:

  • Assists team leader(s) to ensure that non-clinical tasks are completed/followed-up during and from previous shift(s)

  • High School Diploma required or equivalent. Office Administration certificate/diploma preferred. Knowledge of Medical Terminology preferred.

  • Proficient in MS Office Suite, EMR, and data entry/keyboarding skills

Clinical Extern – Nursing (Temporary Part-Time)

Position summary:

The clinical extern will be working in the capacity of an unregulated health care provider. Nursing students are welcome! Clinical Externs have access to dedicated Extern Mentor Coordinators who provide comprehensive orientation, ongoing education and support. Clinical Externs work in collaboration with a unit nursing team under direct and/or indirect supervision of an appointed regulated healthcare professional (RHP). Eligible Clinical Externs-Nursing students may perform pre-identified Controlled Acts authorized to nursing within the Regulated Health Professions Act (RHPA) of Ontario delegated to them under the supervision of a RN or RPN who is authorized to perform them. Externs will be hired on contract, during which the Extern will work part time hours to a maximum of full-time hours (37.5 hours per week). The scheduling of these hours will be determined based on unit needs and may include day/evening/night and weekend shifts.

Primary Responsibilities:

  • Practices under the direct and/or indirect supervision of the Registered Nurse, Registered Practical Nurse, or regulated health professional (RHP) assigned to them on the unit and in a manner consistent with Runnymede policies standards, while taking into account CNO guidance for unregulated care providers
  • Works with the supervising RHP to ensure they have the knowledge, skill and judgment needed to perform all actions/duties, under direct or indirect supervision such as patient assessments, measuring vital signs, assisting with ambulation and activities of daily living (toileting, bathing, feeding), completing documentation of care provided, and other care activities necessary in meeting the patient’s personal needs and comfort.
  • Provides assistance with lifting, turning, transferring, transporting and ambulating as per the hospital policy.
  • Provides measures to ensure skin integrity and reduce pressure injuries.
  • Assists with comfort measures/patient safety by ensuring environment is clean, tidy, and free of hazards.
  • Other duties may include setting up patient rooms to receive patients, assist with admissions, transfers and discharges, intentional-rounding, responding to call bells, and performing safety checks.

Patient Flow Coordinator – Discharge (Permanent Casual)

Position summary:

An integral member of the interprofessional team, the Patient Flow Coordinator – Discharge is key points of contact for discharge planning to formulate, facilitate and coordinate discharge plans focused on the needs and goals of the patient and their family/caregiver. The primary goal of Discharge Planning services is to support and enable patients and families to make timely transitions through the healthcare system with the goal of providing an exceptional patient and family experience.

Primary Responsibilities:

  • Coordinates and develops a timely, effective and safe discharge plan in collaboration with the patient and family/caregiver, and interprofessional team members.
  • Works with the clinical team to identify high risk patients and those patients with long length of stay, Alternate Level of Care (ALC) designation, and multiple treatment issues to coordinate early intervention with respect to discharge planning.
  • Documents interventions in a format that facilitates the monitoring and evaluation of short and long term effects of case management; ensures records are accurate and up to date.
  • Evaluates and revises the discharge plan in collaboration with the patient, family/caregiver and interprofessional team to ensure that patient care needs are met.
  • Organizes and leads family care conference to facilitate patient discharge. Creates follow up plan from care conference.
  • Identifies and coordinates community resources from within the public, private, charitable and non-profit sectors for the purpose of coordinating patient care plans post hospital stay, collaborates with Social Worker as required.
  • Acts as a resource to family/caregiver and interprofessional team members regarding community resources and the discharge planning process.
  • Maintains knowledge of local, provincial and federal guidelines impacting the discharge planning process.
  • Identifies trends that impact on the discharge planning process
  • Closely monitors discharges on a day-to-day basis and makes recommendations to clinical leadership team to manage bed allocation and utilization.
  • Works collaboratively with the leadership team to develop strategies to improve and enhance discharge and flow.
  • Utilize tools to monitor the discharge planning process, including collection of data pertinent to Hospital’s discharge provision.

  • Post-secondary education in a relevant healthcare field.
  • Relevant Home and Community Care Support Services experience preferred.
  • Current registration with the appropriate regulating college preferred.

Position Summary:
