How To Make An Access Request To General Records Of The Hospital

To make an information request, please use the hospital’s Access to Personal Information and Correction of Personal Information Request form.

Completed Forms Should Be Sent To:

Information and Privacy Office
Runnymede Healthcare Centre
625 Runnymede Road
Toronto, ON
M6S 3A3

There is a small administrative charge to place a request for information. Therefore please include a cheque or money order in the amount of $5 payable to Runnymede Healthcare Centre. Please do not mail cash.

The hospital reserves the right to apply fees to information requests, as prescribed by regulations, for the cost of every hour of manual search required to locate a record, the costs of preparing the record for disclosure, computer and other costs incurred in locating, retrieving, processing and copying a record, shipping costs and other costs incurred in responding to a request for access to a record.

When the amount to be paid is estimated to be over $25, the hospital will provide the requestor with the estimate of the cost before giving access to a record.

If you have any questions about the information access request process, please contact the information and privacy office at info.privacy@runnymedehc.ca or 416-762-7316, ext. 2117.
